BEFORE YOUR APPOINTMENT


  • Complete and return a history form to our email for each pet (either in WORD or PDF format) 
  • Read, sign and submit the client contract. Carefully read the client contract before signing it. It outlines exactly what is included with your initial consultation fee, and how we handle and charge for ongoing care  
  • If your pet is reactive (barks, growls, snarls, lunges, or bites), sign and submit an aggression waiver 
  • After you have scheduled your appointment, and we have received all completed forms, we will add you to our call-back list in the event of an earlier cancellation

APPROPRIATE FACE COVERINGS (MASKS) ARE REQUIRED FOR EVERYONE WHILE INSIDE OUR OFFICE, WHICH WE WILL PROVIDE. MASKS MUST BE WORN CORRECTLY, COVERING YOUR NOSE AND MOUTH, FOR THE DURATION OF YOUR APPOINTMENT.

​Examples of safe and effective face coverings include N95 respirators with no valve, disposable polypropylene or cotton surgical masks, and homemade cloth masks with multiple layers of fabric.

​Bandanas, handkerchiefs, knitted masks, fleece balaclavas, neck fleeces (gaiters), and masks with exhalation valves are not acceptable face coverings. We are happy to provide a safe facial covering for you at your visit.  There are no exceptions to this policy.
