BEFORE YOUR APPOINTMENT


  • Complete and return a history form via email (either in WORD or PDF format) 
  • Read, sign and submit the client contract 
  • If your pet is reactive (barks, growls, snarls, lunges, or bites), sign and submit an aggression waiver 
  • After you have scheduled your appointment, and we have received all completed forms, we will add you to our callback list in the event of an earlier cancellation
    • Appointments fill quickly; if you would like to be considered for an earlier appointment, please answer if we call you

APPROPRIATE FACE COVERINGS (MASKS) ARE REQUIRED FOR EVERYONE WHILE INSIDE OUR OFFICE, WHICH WE WILL PROVIDE. MASKS MUST BE WORN CORRECTLY, COVERING YOUR NOSE AND MOUTH, FOR THE DURATION OF YOUR APPOINTMENT.

​Examples of safe and effective face coverings include N95 respirators with no valve, disposable polypropylene or cotton surgical masks, and homemade cloth masks with multiple layers of fabric.
