• Complete and return a history form via email (either in WORD or PDF format) 
• Read, sign and submit the client contract 
• If your pet is reactive (barks, growls, snarls, lunges, or bites), sign and submit an aggression waiver​ 
• After you have scheduled your appointment, and we have received all completed forms, we will add you to our callback list in the event of an earlier cancellation
  
  • Appointments fill quickly; if you would like to be considered for an earlier appointment, please answer if we call you
